  • Research summary

Almost 40,000 hysterectomies (surgical removal of the uterus) are performed for benign disease each year in the UK. Laparoscopic 'key-hole' hysterectomy is being increasingly done in favour of conventional ‘open’ hysterectomy. This paradigm shift has been driven by advances in surgical technologies and improved training and the belief that recovery is quicker because large surgical incisions are avoided. There has been recent interest in the feasibility of ‘same day discharge’ (SDD) after laparoscopic hysterectomy because it is minimally invasive with less post-operative pain. Moreover, earlier mobilisation and reducing the length of stay in hospital may be desirable to prevent complications such as venous thrombosis and nosocomial infections and utilise scarce health care resources more efficiently. Despite the increase prevalence of laparoscopic hysterectomy and the potential advantages of shortened hospital stay, SDD is not an established model of care. Moreover, where SDD is practiced, compliance with care pathways and discharge protocols is not optimal and this reflect conflicting beliefs of health care professionals about the safety and desirability of SDD and the views, expectations and knowledge of patients.

We therefore designed a patient educational website to provide clear, easily accessible, patient friendly information explaining what SDD after laparoscopic hysterectomy is, why SDD may be beneficial and what to expect from decision for surgery, through to the admission to hospital and immediate, short and long term recovery. Our pilot randomised controlled trial (RCT) will evaluate the potential efficacy of this innovation in enhancing compliance with the same day discharge and the explore the views of users regarding the usability and value of the educational website.
