    By 2050, the number of Europeans over 65 years old will increase considerably. Those aged 65 years or over will account for 29 % of the EU-28’s population by 2080, compared with 18 % in 2013, according to Eurostat predictions (Eurostat 2014). Three issues require attention, i.e. (a) prescription and adherence, (b) falls prevention, (c) frailty. Although distinct, these problems are all interrelated. Older persons may have multiple, interacting health and social problems. In the Urban Health Centres Europe (UHCE) project an integrated health and social care approach for community dwelling senior citizens will be evaluated in five sites (Rotterdam, Rijeka, Manchester, Valencia and Pallini). This part of the study implements and evaluates the approach in Manchester.
