    Summary of Research
    As we get older, we may feel less able to be active and may struggle with everyday tasks because we feel more weak and tired. For some people this feeling is much worse. These people are called frail. Once people are aged 80 years and over, between a quarter and a half will show some of the signs of frailty. These people get admitted to hospital more often and may have to be looked after by others in a care home.

    We know that older people can be helped to cope with frailty. Help can be given that may slow down the way frailty affects peoples’ lives, meaning that they can keep doing things themselves, and living a good quality of life. To do this, people need support at home from health services provided by their family doctors or local nurses.

    Our research will look at ways in which we can better support people at home and how community nurses should provide care that picks up signs of frailty early. The first part of our project asked nurses about their thoughts on the important parts of care for frail older people. This helped us develop a care plan, based on the individual people’s needs, that can be used by nurses together with patients and those that look after them at home.

    In the second part of our research, we want to do a small study that will test this care plan. There will be one group of patients who will use the new care plan and a group who will not. In addition, we will talk to frail older people, carers and nurses to learn more about their experiences of taking part in this study. The information that we collect will help up plan a future larger study.
