Research summary
Background: In patients with cancer, the prevalence of chronic pain is estimated to be more than 70%.Undertreatment of cancer pain reduces patients’ quality of life and emotional well-being, enhances burden, and increases healthcare service use and costs. Inadequate standardised processes, training and reporting systems prevent optimal management of cancer pain. Little is known about what triggers pain assessment and management to occur in oncology outpatient service (OS) practice, this affirms the need for a standardised approach to cancer pain management.
Aim: to evaluate the feasibility of [1] implementing standardised pain management into routine oncology outpatient services and [2] conducting a cluster randomised trial in three secondary care oncology centres in the North of England.
Methods: 12 oncology outpatient services across three tertiary oncology referral centres in Leeds, Hull and Sheffield will be recruited. Randomisation (1:1) will occur at service-level to receive the intervention (EPAT) or control (usual care). Patients with cancer will be recruited from within participating OS to provide patient reported outcome data and medical notes review data.
Output: Implementation of effective pain management processes within oncology OS has yet to be successfully achieved. Patients in the intervention arm may benefit from earlier access to analgesia, receiving advice for pain and medicines and timely access to palliative care. If indicated, we will submit an external funding bid in 2023 for a definitive phase-III randomised controlled trial (RCT).
