    Osteoarthritis (OA) is the most common form of arthritis and remains one of the few chronic diseases of ageing for which there is little, if any, effective treatment. Treatment development has focused on protecting cartilage from damage despite the absence of pain fibres in cartilage. Many patients with knee OA have inflammatory changes within the synovium of the knee. Intra-articular corticosteroids reduce inflammation and have been used with benefical effect in knee OA. We plan to study 120 patients with osteoarthritis and knee effusion who are treated with intraarticular steroids and look at inflammatory changes in the knee (using magnetic resonance imaging) before, after the injection and at the time of failure (when pain recurs). The aim of this study is to determine whether patients with inflammatory change in their synovium respond better to steroids than those who do not, and whether change in the degree of inflammation (assessed by MRI) following injection and on recurrence of pain correlates with changes in the level of pain
