    This study sets out to assess if the early detection of cognitive decline (memory problems) following a recent stroke or transient ischaemic attack (TIA or mini-stroke) can be prevented, slowed down or potentially reversed by addressing vascular risk factors (VRF) more intensively.

    Vascular risk factors (e.g. high blood pressure, abnormal lipid levels, diabetes and atrial fibrillation) can contribute to the cognitive decline potentially resulting in vascular dementia as well as in mixed type dementia (Alzheimer’s and vascular dementia).

    All adult patients admitted to the participating hospitals with confirmed stroke (first/recurrent) or TIA will be considered for the trial.

    The initial study will assess the feasibility, acceptability and safety of an intervention which aims at controlling risk factors more intensively. We will estimate the potential impact in terms of clinical and cost effectiveness and the sample size for a future multicentre definitive study in the NHS setting.
    A parallel observational study will be carried out to see if there is a link
    between the number of vascular risk factors, their level of control and
    subsequent cognitive function after stroke/TIA. The study has the potential to
    influence clinical practice and improve the quality of life in this high risk patient population.
