    High blood pressure (BP) is the largest cause of morbidity and mortality worldwide. With the advent of non-drug intervention for high blood pressure, investigating those who would benefit from procedural intervention for the disease has never been of higher clinical relevance. As clinicians, we must differentiate between patients who have high BP despite therapy and those not compliant with therapy. Different interventions are available for the 2 groups and failing to diagnose the second issue could lead to unnecessary investigation and harm. Patients with treatment resistant high blood pressure, those on three or more tablets and still not achieving adequate BP control, will undergo supervised drug administration. This would involve taking their usual prescribed medication under observation and subsequently undergoing ambulatory blood pressure monitoring(ABPM). These results will be compared to previous ABPM results, undertaken during there normal clinical practice. Armed with this data, if there is a significant drop in BP following observed therapy we will review incorporating the practice of directly observed therapy into our normal clinical practice in Lothian.
