    The Collaboration for Leadership in Applied Health Research and Care Northwest London (CLAHRC NWL) is conducting an evaluation of the FRAILsafe intervention which aims to improve safety for frail older patients in acute medical units. Led by Sheffield Teaching Hospitals NHS Foundation Trust on behalf of British Geriatrics Society, the FRAILsafe intervention focuses on a series of measurable harms posing a risk to older patients, including confusion, falls and adverse drug reactions. The project aims to implement an evidence-based checklist to improve communication, team working and prompt reliable care, and to test its effective implementation, with a view to improving quality of care for frail older people who are admitted to hospital with a medical emergency. In the first year of the project, implementation will be carried out in 12 acute medical units across the UK, while hospital teams will also be supported by a Breakthrough Series Collaborative, which is a short-term learning system that brings together a large number of teams from hospitals or clinics to seek quality improvement in a focused topic area.

    As an evaluation partner, CLAHRC NWL is aiming to support the delivery of the project, capture its achievements and contribute to its long term success. The primary objective is to provide formative feedback regarding the process of implementation and to identify achievements, barriers and facilitators of change in a timely manner. The secondary objective is to understand the impact of the intervention, including the impact on existing systems and processes of care and changes in quality of care and patient outcomes.
