Research summary
Each year about 30,000 people receive resuscitation for an Out of Hospital Cardiac Arrest (OHCA) in the United Kingdom (UK) from which only about one in twenty people survive to go home from hospital. Information collected by the Department of Health has shown that the vast majority of people suffering an OHCA and are transported to hospital before gaining a return of spontaneous circulation (ROSC) do not survive. This project will try to find out whether death can be predicted, in order that resuscitation attempts can safely be terminated on scene. It will then create a clinical decision rule for ambulance crews to use. The study will look back at data already gathered by the Trust from resuscitation attempts to find what factors, if any, can be used to predict death. We will create a clinical decision rule from these data. We will then validate the rule, by applying the rule to subsequent resuscitation attempts. At this stage of the study, ambulance crews will transport patients according to existing policy and procedure. Following transport they will complete a form, containing the elements of the clinical decision rule, to determine whether this rule will have resulted in transport or termination of efforts on scene. Patient outcomes will be established using data currently gathered by the Trust for purposes of Ambulance quality indicators.
