Research summary
There is a great deal of current interest in enhanced recovery after surgery (ERAS) protocols. It is suggested that he benefits of this multimodal approach can be attributed to attenuation of the surgical stress response. Epidural analgesia (EA) is considered an important part of ERAS protocols however they are associated with significant complications and raise logistical issues to the provision of postoperative care. A recent study in our organisation demonstrated EA conferred no benefit with regard to length of stay over continuous wound infusion (CWI) of local anaesthetic following major colorectal surgery. CWI does not however provide optimal intraoperative or early postoperative analgesia. We aim to combine CWI with spinal anaesthesia and compare this technique with CWI alone. We hypothesise that this will provide optimal intra and postoperative analgesia and will attenuate the surgical stress response to a greater degree.
