    The NHS Long Term Plan states that every person diagnosed with cancer will have access to personalised care and support, which means that people’s needs and preferences are fully identified and individualised support planned. Personalised Care and Support Planning helps people take an active role in the way their care is planned and delivered. People are offered supportive conversations, along with assessment of holistic needs, to take into account their broad life context.

    There exists an array of resources to support the personalisation of care, such as tools to assess need and tools to record action to be taken. However, there is evidence that assessment of need and action planning are not conducted systematically. The Wessex Cancer Alliance has commissioned the Centre for Psychosocial Research in Cancer (CentRIC) to undertake a service improvement initiative, through the development and feasibility testing of a systematic approach to personalised care needs assessment and support planning. The Personalised Assessment and Care in Cancer (PACC) pathway has been co-produced with experts in cancer care, and patient representatives, to build on and align with current policy and practice. The purpose of the current study is to investigate the feasibility of the pathway in clinical practice, considering both health care professional and patient views, and to gather some information to support a future clinical trial of the pathway. The study will be conducted in 2 NHS cancer services in England. Patients who experience the pathway will be invited to complete a short questionnaire and to take part in an interview about their experience. NHS staff who are involved in the delivery of the pathway, as well as a wider group with knowledge of the pathway, will be invited to take part in an interview.
