    High blood pressure (BP) is the leading modifiable risk factor for cardiovascular disease and is implicated in half of all strokes and heart attacks. One-third of adults in Scotland have high BP yet only one-quarter of these have their BP controlled to target (<140/90 mmHg).

    Reasons for the low rate of control are varied but include individuals lack of confidence, knowledge and understanding of the condition and treatment plan, not taking or missing medication and infrequent monitoring. These issues have been worsened by the ongoing COVID-19 pandemic which has brought changes in routine BP screening, access to medical care and chronic disease management in GP and hospitals, shielding of the highest risk groups and a change in health and research focus. Empowering patients to have a better understanding of their condition and becoming actively involved in the monitoring and management may lead to improved satisfaction, improved BP control and health outcomes and reduction in the use of GP/hospital BP clinics.

    OPTIMA-BP will recruit 200 participants attending the Glasgow BP Clinic and randomly split them into two groups. One group will receive a home BP monitor and access to an online BP education portal whilst the other receives a home BP monitor but will not have access to the portal. Participants will be asked to measure their BP at 2-monthly intervals. Questionnaires will be done at 6-monthly intervals assessing blood pressure knowledge, medication use, lifestyle factors, quality of life and health literacy. The study will last 12 months.

    OPTIMA-BP aims to assess the knowledge and understanding of high BP in those attending the Glasgow BP Clinic; establish and evaluate the use of a web-based education portal; assess the acceptability of using home BP monitoring; establish if using home BP monitoring with the education portal improves BP control and quality of life.
