    This is a mixed methodology evaluation of a healthcare design / service improvement innovation.

    The overall aim of Making Waves is to spread the model of a community COPD (chronic obstructive pulmonary disease) clinic to six further local health economies within the East and West Midlands Region, also evaluating the original site in Coventry.

    The community clinic model brings together respiratory clinicians, voluntary sector facilitators, volunteers, with people with COPD and their carers to share insight and good practice in self-management and living with COPD.

    The focused objectives of the service for people with COPD are:
    • Increased mental well-being.
    • Improved ability to self-manage.
    • Improved take up of low cost/ highly effective treatment e.g. smoking cessation and pulmonary rehabilitation.
    • Reduction in unplanned admissions.
    • Emergence of a new model of service.

    The evaluation will use mixed methods to evaluate the performance of the seven sites against these objectives. The aim is to report on the feasibility of the community clinic model, to support the spread of effective innovation with a co-created addition to the standard range of support available for people with COPD.

    Participants in the clinic will be asked to complete a small pack of self-assessment validated instruments. These are:
    • EQ5D (assessing quality of life)
    • COPD Assessment Test (CAT)
    • LTC6 (assessing health status in living with a long-term condition)
    • Warwick-Edinburgh Mental Wellbeing Scale (WEMWBS)
    • MRC Dyspnoea Scale (MRC)
    • Hospital Anxiety and Depression Scale (HADS)/ Generalized Anxiety Disorder 7 (GAD7)
    • Patient Activation Measure (PAM)

    In addition, participants will be asked to complete a short questionnaire on their usage of healthcare facilities.

    Participatory workshops will be held with participants within the clinics in Spring and Summer 2017.

    Economic analysis will take place utilising data gathered above.
