    The aim of study is to evaluate the feasibility of assessing currently used programme outcomes (standard audit data) over a longer term than currently being performed at the Knowsley Cardiovascular Disease Service. The study will collect information 8-14 months after patients` discharge from the rehabilitation sessions.
    The study will use both past clinical records and a new assessment utilising the questionnaire used by the National Audit of Cardiac Rehabilitation (patients will be familiar with this questionnaire as it is used at the beginning and discharge from the rehabilitation sessions).

    The questionnaire focuses on the following:
    1. How much does cardiovascular rehabilitation influence patients` aspiration to continue exercising as an outcome of exercise component of the programme?
    2. What is patients’ awareness about their individual state of health and disease?
    3. What is the extent to which patients continue to maintain healthy lifestyle including smoking cessation, the level of alcohol consumption, performing daily activities?
    4. What is patients` perception of their physical disease or post event (PCI, surgery)?
    5. What is patients’ condition of psychological health?

    The target participants are patients who have finished cardiovascular rehabilitation programme 8-14 months ago in April - November 2013. The questionnaire will be sent to 197 patients by post; a 60% response rate is predicted to yield an estimated sample size 118. Data collection will be finished by the 1st of August 2014.
