Research summary
The management of people with stroke is moving towards a patient-centered integrated care approach. However there is currently no consensus and no guidance on how best to deliver this model of care. This can result in fragmented and inconsistent healthcare, inequalities, communication issues and patient frustration when they are not involved in decisions over their care. This research will aim to bring together the findings of published literature, patient experiences and patient and healthcare practitioners’ views on what an integrated stroke pathway should look like. This will be done through a systematic review of the published literature to identify components of integrated care that benefit patients, a series of focus groups involving stroke survivors to listen to their experiences of recovery and find out what really matters to them, and a Delphi survey to get the opinions of healthcare professionals and patients to identify what components should be included in an integrated care pathway and how it should be delivered. The research will allow the creation of a streamlined and patient-centered pathway that will ultimately improve care and outcomes for people with stroke.
