Research summary
Crohn’s disease is an inflammatory condition that can affect any part of the gastrointestinal tract from mouth to anus. Around 1 in 3 patients with Crohn’s disease will develop problems around their anus. The most challenging form of these to challenge is a fistula (connection between anal canal or rectum and skin of the buttocks). These can be very difficult to treat with as few at one in three patients achieving long term healing.
There are a number of surgical options for the management of perianal Crohn’s fistula. These include placement of a rubber band for drainage (seton), fistula plug (placement of a collagen plug to close the internal hole of the fistula), and advancement flap (moving part of the lining of the anal canal to close the hole). There are situations where more than one option could be considered in the management of this condition. The selection of procedure reflects the opinion of the surgeon and the priorities of the patients.
Shared decision making (SDM) is a process involving sharing of information and supported deliberation6. This approach is associated with improved patient satisfaction with care. One of the principle quality markers of tools to facilitate SDM is ‘providing information’. In order to understand what information should be provided, we would like to undertake semi-structured interviews of patients who have previously undergone surgery for this condition. These interviews will explore information given about surgery, information desired with the benefit of hindsight, and desired modality of sharing information.
