Research summary
Sudden cardiac death is very common in patients with End Stage Renal Failure on dialysis. From clinical experience, this occurs mostly at night.
On the background of this there are several hypotheses and facts, including circadian variations to tissue sensitivity to glcocorticoids and insulin gene and protein expression, autonomic dyssynchrony leading to early morning hypoglycemia, expression of CLOCK proteins, etc.
We hypothesize that nocturnal hypoglycaemia and arrhythmias may be the cause for clustering of sudden deaths at night in HD patients receiving standard dose HD.
We will study 20 HD and 20 CAPD patients, who are non diabetic and have no cardiac disease. Data collection will be done by continous glucose monitoring system for 7 days and ECG Holter monitoring for 7 days simultaneously.It is a pilot study.
Primary Endpoint
• Frequency of hypoglycaemia (glucose <4mM) between the hours of 2200 – 0800.
• Longest QTc interval every night between the hours of 2200 – 0800.- Secondary Endpoint
• Average glucose concentration between the hours of 2200 – 0800 in HD vs PD patients
• The frequency of individual cardiac rhythm disturbances.
• The association between disturbances in cardiac rhythm and the haemodialysis procedure.Duration of the study is one year, from 1/1/2013 to 1/9/2013 data collection and procedures, till 31/12/2013 complete write up of project and submission of manuscript to journal.REC name
