Research summary
A mixed methodology will be used to examine in-patient suicide whilst under non-routine observation, in the UK. 2006-2012 cases will be identified from the existing National Confidential Inquiry into Suicide and Homicide by People with Mental Illness [NCISH] database. Additional information will be obtained through a number of data sources, such as a web based survey, Serious Untoward Incident [SUI] reports and focus group discussions with nurses and psychiatrists.
The aims are to:
1) describe the characteristics of patients who died by suicide whilst under non-routine observation
2) determine the antecedent leading to the suicide, including staffing levels, ward design, and observation policy and practice
3) identify problems with observation policy and practice that may have contributed to these deathsThe study will involve the following:
• an open call to patients and clinicians to share their views and experiences of non-routine observation via a web based survey. This will inform the development of a proforma.
• collection of SUI reports relating to the in-patient suicides identified through the NCISH dataset. Data will be extracted using a proforma. Additional documents from NHS Trusts will be requested; for example monthly incident reports, staffing information for the ward at the time of the suicide and Trust observation policies.
• focus groups with nurses and psychiatrists to discuss aspects of observation policy and practice.
This study will inform clinical policy and practice to reduce patient suicide on in-patient psychiatric wards. The key messages for services will be highlighted in a report and will form part of the NCISH toolkit for use throughout the service.
