Research summary
This is a two arm prospective study focusing on investigating signs and symptoms of advanced chronic kidney disease in older adults involving a dietetic-led integrated and collaborative model of care, with the aim of cost effectively improving patient-centred and medical outcomes. The patient-centred outcomes include quality of life, depression and self-management skills. The medical outcomes include hospitalisation rate, survival, dialysis free survival time and healthcare costs. The two arms of the study involve
1) a cohort of 120 patients who will be monitored every 3-6 months for 24 months. These patients will be receiving standard care.
2) a feasibility study arm that will involve an additional 30 participants who will be exposed to a dietetic-led integrated and collaborative model of care in addition to their standard care.
The outcomes of these two study arms will be compared to see if there is any benefit to the dietetic-led integrated and collaborative model of care. In addition the cohort study will provide valuable information on the trajectory of advanced renal disease in older people.REC name
