    Background: This is Phase 3 of a mixed-methods process evaluation investigating the implementation of the Goal setting and Action Planning (G-AP) framework in 3 community rehabilitation settings. G-AP is a theory and evidence based framework to support person centred goal setting practice.
    Study aim: To investigate the implementation of the G-AP framework in community rehabilitation settings. G-AP implementation outcomes and how these outcomes were reached will be investigated from the perspective of rehabilitation patients, staff and managers, one year on after the G-AP training was delivered.
    Study design/ setting: Participants will be recruited from three community rehabilitation teams in NHS Lanarkshire. Data collection is anticipated to last 3 months; the full study including data analysis is anticipated to last a total of 6 months.
    Participant involvement/ data collection:
    I. Rehabilitation staff: questionnaire and focus group;
    II. Rehabilitation team managers: semi-structured interview
    III. Patients receiving rehabilitation services: semi-structured interview.
    IV. Team documentation: Documentation that describes the team context and/or documents usual goal setting practice used within it will be also be collected and analysed.
    Benefits / intended impacts for participants are anticipated as follows;
    • Rehabilitation staff / managers: Delivery of evidence based goal setting practice, local tailoring of a structured person centred goal setting approach; person-centred care guidelines fulfilled. As well as creating an opportunity to engage with the research process, this project will help participating teams think through the outcome of the G-AP implementation process and how these outcomes were reached.
    • Rehabilitation patients: Patients with a variety of neurological conditions and brain injury (not only stroke patients as in previous G-AP research) will be supported to set and pursue personal goals using an evidence based framework; Improved patient satisfaction and experience of care.
