Research summary
Chronic osteomyelitis is a challenging problem. Its treatment will often require multiple debridements, prolonged inpatient stays and long courses of antibiotics. Options for the treatment of chronic osteomyelitis include long term antibiotics and surgery. Recently a trend toward single stage treatment rather than multiple debridements has been reported with good results. The basic principles of surgical treatment of osteomyelitis involve radical debridement, eradication of the dead space (cavity) and definitive wound coverage. Tissue dead space can reduced with the use of muscle flaps or antibiotic loaded implants such as Stimulan beads.
We would evaluate patient records for two, single stage osteomyelitis treatments. Initial evaluation would be between debridement and flap versus debridement and absorbable antibiotics calcium sulphate pellets. This evaluation would be limited to all treatment done at the time of index osteomyelitis surgery. This evaluation will include length of stay, antibiotics and all associate potential costs to the hospital.
Health care economics can be used to investigate how health care systems function and to evaluate the costs, charges and expenditures associated with the treatments available.
This study aims to compare the health economics and therfore cost comparison involved in two different single stage treatments of chronic osteomyelitis - absorbable calcium sulphate pellets versus one stage debridement and muscle flap coverage. The best performing treatment will then be compared to historical data for 2 stage osteomyelitis treatment.
