    Patients with repetitive breathing difficulties during sleep together with interruption of their sleep pattern have a condition called obstructive sleep apnoea (OSA). OSA has an effective treatment that aims at preventing the closure of the airways during sleep, the so-called continuous positive airway pressure (CPAP) treatment.

    OSA has been associated with a high risk of heart disease. In addition, there is a growing body of evidence confirming the link between OSA and the risk of developing diabetes. Given this association, a clear screening strategy is required in patients with OSA to detect abnormalities in their glucose levels.

    We aim to look at patients, not previously known to have diabetes, referred for OSA assessment (fifty with confirmed OSA and 50 weight-matched controls, referred but found to be negative for OSA). In addition to checking for glucose status with the ’gold-standard’ test - the glucose tolerance test (drinking a sugar drink and checking the blood sugar levels) - we will add the following:
    1. Repeat glucose tolerance test (to ensure the two results correspond with each other, in view of reports of changes when the test is repeated)
    2. Continuous glucose monitoring system (CGMS); a device currently used in managing people with existing diabetes. A small sensor is attached under the skin via a fine cannula to record the sugar levels every few minutes for up to 5 days.

    We believe that the CGMS will yield more accurate real-time data and allow more definitive assessment of the presence of diabetes in this high risk group.
