Research summary
After a hospital admission some people may need additional care or rehabilitation. This can be provided in people's own homes, or within 24-hour bedded facilities. In England the Discharge To Assess model for discharging patients from hospital has been introduced nationally since 2020. The aim is to support patients so that they can be safely discharged, with rapid assessments and quick access to care and support (if needed). However there has been little exploration of patient, caregiver and staff perceptions of this model of practice.
The Discharge to Assess model aims to support people to leave hospital as soon as clinically ready with the provision of appropriate recovery support. The model is based on 4 different discharge pathways 0 (returning home with no additional support), 1 (returning home with provision of additional health or social care), 2 (discharged to 24-hour bedded care setting for further recovery and rehabilitation), or 3 (permanent new admission to 24-hour care). Using the Discharge to Assess Model most people who require care support on discharge would be discharged on pathways 1 or 2 and would then have their longer-term needs assessed in the community after a period of recovery. This research study aims to speak with people who have recently been discharged from hospital on Pathways 1 and 2, their caregivers and staff working within discharge services, to explore their perceptions of the discharge process.REC name
