Research summary
The aim of the research project is to explore the effectiveness of new models of integrated care for frail older people, in order to build on existing international and national evidence and address key transferable issues. In order to address these issues, a case study of Lincolnshire West Clinical Commissioning Group’s (CCG) approach to integrating care for frail older people will be used. This involves the development of an integrated frailty pathway designed to pro-actively manage frailty, improve quality of later life, maintain people living independently and safely at home, and ensure timely discharge following any hospital admission.
The project will investigate the impact of the integrated frailty pathway, evaluate the barriers and facilitators to working within an integrated team, and examine the implications of the move toward system-integration.
