    Chronic obstructive pulmonary disease (CCOPD) is a common and preventable lung disease. People with COPD commonly experience breathlessness, cough and sputum production that tend to progress overtime and often deteriorate rapidly, requiring additional treatment. These episodes of deterioration are called exacerbations and are a common reason for hospitalisation in the UK and contribute to the p[progressive nature of COPD. As such, health services have been developed to support self-management and optimise treatment to prevent exacerbations and, when they occur, enable early identification and timely treatment with the aim of preventing the need for hospitalisation. One such service is the Lenus Health COPD Support Service which is provided for patients across Hull and the East Riding of Yorkshire through Hull University Teaching Hospitals NHS Trust (HUTH). This services utilises a digital platform for recording of clinical information, patient interaction with the care-team through direct messaging and remote monitoring of patient reported outcomes measures and physiological data obtained through wearables (e.g. fitbits). This clinical service has been operating since March 2023.

    We will undertake an observational study to compare the outcomes for COPD patients enrolled in the HUTH remote COPD management service utilising the Lenus COPD Support Service with those of matched COPD patients managed without remote support. Routinely collected NHS data will be pseudonymised by the clinical service providers prior to sharing with the research team through the Hull Health Trials Unit Data Safe Haven. Propensity score matching will be used to match cases (patients enrolled on the Lenus COPD Support Service in HUTH) with control patients from i) historical COPD patients admitted to HUTH during the year prior to the Lenus COPD Support Service starting and ii) contemporary controls from a neighbouring hospital trust (North Lincolnshire and Goole Hospitals NHS Foundation Trust (NLaG).
