The study is to evaluate the effectiveness of the SDEC (Same Day Emergency Care)/HF (Heart Failure) clinic in reducing admissions among heart failure patients presenting to the SDEC.

It will review records of patients admitted primarily with symptoms of Heart Failure (HF) over the period of February to September 2022 and 2023 to look for readmissions with heart failure symptoms within the 1, 3, 6 months following their admission. The 2022 period shall be prior to the SDEC/HF clinic starting and 2023 when the clinic was established.
