Research summary
10-15% of all women have a pregnancy complicated by placental dysfunction. It has long been recognised that one of the biggest risk factors for placental disease is the presence of placental disease in a previous pregnancy, with approximately 25% of all women with a history of placental disease, having a recurrence in a subsequent pregnancy. However, this 25% estimation is only helpful on a population level, therefore we currently treat all women with a history of placental disease in the same way, and offer them the same package of increased antenatal surveillance. This results in unnecessary scans in those women who do not develop recurrent disease, and increases the likelihood of unnecessary intervention. We propose that by examining data from retrospective cases of recurrent placental disease (both index and any subsequent pregnancies), we can calculate risk on an individual level. This would allow us to provide a more tailored package of care, targeted to those who really need it. This would have numerous health economic advantages through improved health resource allocation and a reduction in unnecessary intervention.
