Summary
Current research suggests that people with severe memory problems may receive poor and fragmented care. For example, symptoms such as pain, agitation or low mood may not be recognised and may be poorly treated. In addition, family members and carers of people with severe memory problems may find this challenging or upsetting at times. They sometimes find it difficult to have the time to look after their own needs. Therefore, we are interested in evaluating an enhanced service designed to improve the care that is provided to people with severe memory problems residing in care homes and the support that is provided to family members and carers. The enhanced model of care involves a care leader who will help bring together medical and social care professionals who are looking after those with severe memory problems in order to better coordinate their care. This model of care is also designed to improve the support that is provided to family members and carers of those individuals with severe memory problems and provide training and educational opportunities for health and social care professionals and carers to improve knowledge. In order to do this, a pilot study will be conducted during which the care leader will be introduced into care homes for 6 months where all residents with severe memory problems will receive this enhanced integrated care. This pilot study will provide opportunities to learn and understand how it might operate in practice and to obtain data to inform a future definitive trial.
