Research summary
This 4 year European project will develop, implement, trial and evaluate the C3-CLOUD system which is designed to improve the management of patients with multi-morbidity and poly-pharmacy. It will optimise patient education and self-management, foster closer collaborative working between healthcare professionals, and encourage shared decision making between patients and professionals.
The project is funded by the EC and led by Warwick University. The UK arm of the study is sponsored by South Warwickshire NHS Foundation Trust (SWFT).
There are 12 European partners (in UK, Spain, Sweden, France, Finland and Germany) involved in this project including technical, clinical and research partners.
The study will run for 15 months in 3 pilot sites - Basque Country in Spain, RJH in Sweden and South Warwickshire in the UK. NB: this application relates to the UK(SWFT) based part of the study only.
The study will focus on patients with 2 of more of the following conditions - diabetes type 2, heart failure, renal failure and depression.
In South Warwickshire, 240 patients and 16 health and care professionals will be recruited to the study. Half of the patients will be randomised to the intervention group, i.e. will use the system, and the other half will be randomised to the control group, and will receive conventional care (without the system).
The system consists of 2 parts. Health professionals will use the C3DP system which collects information about the patient & their conditions in a patient-centric way. The system will also provide clinical decision support tools, using reconciled NICE guidelines. The patients will use the Patient Empowerment Platform where they can view their care plan, manage their goals & activities, access training materials and contact their healthcare professionals etc.
The evaluation will mainly consist of questionnaires and analysis of healthcare usage data. Please see the attached Overview & Description of Action for more details.
