    The aging population and advances in medical science means that people with chronic diseases are living longer. This creates challenges, such as how to: sustain quality independent living; support carers facing increasing burdens of care; and create sustainable health and social care systems given limited resources. Digitally supported interventions help people with progressive advanced chronic conditions maintain or improve their health, reduce deterioration, increase their independence and optimise health resource utilisation. Additionally, the COVID-19 pandemic accelerated the shift towards remote telecare, demonstrating its ability to provide support and care.

    This project, ADLIFE, will deploy 2 digitally enabled health interventions:

    • a Personalised Care Plan Management Platform for clinicians, integrated within clinical sites’ ICT systems, that will follow clinical guidelines to create patient care plans based on each patient’s baseline and most recent clinical data.

    • a Patient Empowerment Platform for patients and carers to view personalised goals and activities from the patient care plan, complete outcome measurements, and receive information about useful interventions that may improve their wellbeing.

    ADLIFE aims to demonstrate outcomes-based efficiency gains in health and care delivery through exploring patients’ health outcomes, quality of life, experiences e.g. autonomy, empowerment, and decision making, as well as resource utilisation and clinicians’ experiences of coordinated care and working conditions.

    Patients, aged 55 or over, with chronic obstructive pulmonary disease and/or heart failure, and their informal caregivers, will be eligible to participate at home for 12 months. Clinicians who provide primary or secondary care for these patient populations will also participate.

    ADLIFE is a multi-centre, European funded project that involves 7 clinical sites and 6 countries. This application details the processes and procedures for the project within the United Kingdom where the ADLIFE platform will be integrated and scaled in the ICT health systems of two NHS sites.
