    The routine treatment of gum disease consists in removing the hardened deposits of dental plaque off the surfaces of the teeth under local anaesthetic using a combination of hand and ultrasonic instruments. This is called ‘non-surgical periodontal treatment’. This procedure can cause sensitivity and recession. Non-surgical treatment is technically challenging due to the complex anatomy of the teeth and instruments may not be able to reach some areas, thus leaving deposits behind.
    In advanced cases, non-surgical treatment is insufficient in treating the disease and patients may need surgery. During surgery we lift the gums to gain better access to the teeth to clean them and in some cases, we may be able to graft some human, animal or synthetic tissue to encourage regeneration of new tissue to support the teeth.
    The Erbium, Chromium: Yttrium – Scandium – Gallium – Garnet (Er, Cr :YSGG) laser is licensed to use in dentistry as a complement to non-surgical treatment. This laser works by removing tartar (hardened dental plaque), killing bacteria under the gum line and improve healing of gums and bone. However, this study is needed to measure its effect and understand how new bone formation following this treatment actually happens. We will be measuring the depth of the gum pocket, any bleeding of the gums, changes in content of gum fluid, as well as x-rays to view changes in bone level.
    We will be recruiting patients with severe gum disease referred to the periodontology department. The patients will be adults with no severe medical conditions and non-smokers. Patients will undergo conventional treatment on one side of the mouth and the opposing side will receive the same treatment with the addition of the laser on one of the most affected teeth. Following initial treatment patient will be reviewed every 3 months for up to 12 months.
