Research summary
The Ambulance Cardiac Chest Pain Evaluation Study in Scotland (ACCESS) was a prospective cohort study conducted in the pre-hospital setting, in the Grampian region of Scotland. Eighty five paramedics from 11 ambulance stations recruited a study population of 1054 patients with chest pain suspicious for a myocardial infarction between November 2014 and April 2018. The purpose of this study was to evaluate different potential methods of paramedic pre-hospital assessment of risk in this population to determine if identification of patients who were at low enough risk to be managed without direct hospital transfer could be feasible.
As a necessary part of the ACCESS study, we collected data/time points regarding patient symptom onset, time of ambulance call, time of ambulance attendance to the patient, and time of ambulance arrival at hospital. During the course of the analysis, the question was raised as to whether the length of these times correlated with the development of a myocardial infarction. This has not been evaluated in the literature and may be an important factor in future pre-hospital risk stratification strategies.
