Research summary
The aim of the research is, firstly, to determine how national policy discourses underpinning community based integrated care have been enacted as programme resources designed to create behaviour change in health professionals and service users. Secondly, the study will aim to evaluate to what extent these programme resources have been successful at meeting both policy maker’s aims and creating changes to wider structures. Policy maker’s aims include holistic patient centred care, streamlined integrated services, improved collaboration between the professions, and effectively tailored self-management. Wider changes created by CBIC, include impacts on health inequalities, professional roles, discourses of caring, and individual/state responsibilities. The purpose of testing outcomes at these two levels of abstraction is that the research is able to interrogate the extent to which policy maker’s programme theory has been successfully implemented, whilst subjecting the theory to further critical analysis. The research methods used include textual analysis of policy documents and organizational documents, interviews with those suffering from long term conditions who are receiving treatment from the service, and health professionals within the multidisciplinary teams delivering care. Interviews with patients will take place in their homes or in a nearby clinic, and interviews with health professionals in their office location. Observations will be conducted of training delivered to health professionals, in a training room owned by the lead provider. Observations will also be conducted of day to day interactions and multidisciplinary team meetings, taking place within the four team offices. Statistical analysis will be conducted on internally collected data, in order to determine impact on patient outcomes and experience, and the extent to which this correlated with certain contextual factors. The research is being funded by the lead provider of a community based integrated care (cbic) model within North East Essex, and will form part of a three-year PhD project.
